Medication Therapy Management (MTM) is a comprehensive review of the patient's medications, and a teaching program to educate the patient on how to take their medications safely. Our Pharmacy Doctors work with each individual patient or their caregivers to ensure their drug regimen is maximized to improve the patient's quality of life. Many of the MediCare Part D plans will cover MTM for certain patients that take multiple medications and have multiple medical conditions. Our MTM program will evaluate all the medications the patient has been prescribed and help them understand the drug related interactions of using multiple medications, this assures the patient and/or their caregiver does not create reactions from overdoses or misuse of their prescriptions.
